Welcome back.

Make a new appointment, reschedule an appointment or ask a question.

FMC is here to serve you. Whatever we can do to help you please let us know. You can request a new patient appointment, contact us about a question you may have, or request your appointment to be rescheduled. You can also find helpful clinic and patient information.

Make a new patient appointment

Once you complete the new patient appointment request, your appointment will also need to be confirmed by text through our Athena Patient Portal or phone call from our clinic team.

Since you are already a registered FMC patient you should have access to your personal Athena Patient Portal that you can schedule an appointment and communicate medical information in a secure manner. Due to HIPPA compliance, FMC never captures medical information through this site, only in person, over the phone, or through your personal Athena Patient Portal.

If you have not completed your personal Athena Patient Portal, you can do so by emailing us a request from the form on this page, call us at the Oak Ridge office, or set it up when you are at one of our clinic sites. If you request it through the contact form on this page then we will email you a personal link so you can complete your login.

Top Questions Patients Ask -

What if I need to reschedule my appointment?

FMC understands that sometimes our schedule changes and we need to adjust. We do ask each patient to make their appointment a high priority because we cannot guarantee when another appointment might open up or if you need to reschedule we might not be able to see you as quick as you need. If you need to reschedule an appointment please call the clinic you have the appointment at, or if Telehealth, the Oak Ridge Clinic, and let us know as soon as possible about the change.

What if I need lab work?

FMC understands that lab work, testing, and imaging are tools to help diagnose your care. FMC provides some lab work at our clinic sites and we have partners like the Oak Ridge Medical and Roane Medical Centers to help with additional support. Your provider will give you instructions on how to take the next step of your lab work. You will need to see a provider to set up your orders.

What is a Telehealth appointment?

Think of a Telehealth visit much like your normal clinic visit but we do the entire appointment over your phone, tablet, or computer. You will have a Telehealth appointment much like you do for a regular appointment but you will carry out the appointment with your doctor on your mobile device or phone. Your provider will call you around the time of your appointment (allow for at least 15-30 minute window from your appointment). (Click here to watch a quick Telehealth instruction video)

What is the Annual Physical?

FMC believes preventive care is an essential part of your comprehensive care. FMC provides a no charge annual physical to review your health so your provider can understand your needs and help you remain healthy. Preventive care helps to identify issues and problems so we can understand your needs prior to issues becoming critical. This is also a time to spend with our team to help understand what medical challenges you might be facing, update our medical information on your file, and make sure we have updated lab work.

What if I need medications or refills?

If you need medications or refills please contact your clinic site, or if a Telehealth appointment, the clinic site you would normally go to for a visit. Please allow us at least 14 days or more to help with refills. Please understand that most medications are provided by companies other than FMC, so we can only do so much to help you ensure you have the meds you need for your treatment. Make sure all your Patient Assistance Program applications are completed and turn in.

What Medical Programs do you offer?

FMC manages many of our patients as they enter specific medical needs such as diabetes and Hepatitis C. If you want more information on our medical programs please ask your provider and they will set you up to begin enhanced treatment and support to better treat your current medical conditions.

Need something FMC can’t provide? Our partners have you covered!

FMC networks with other non-profits through UNITE US to secure food, housing, education, clothes and other social resources to assist our patients. We also partner with ETHRA to provide transportation for our patient to and from clinic sites.

FMC can’t do it all. If you need other social services maybe one of these groups can help you.
